Complete an evidence-based gap analysis that considers the needs of those who will experience the provision of Healthcare services in the future (LO1,LO2), global and local drivers of change(LO3), and the needs of your own professional development and those of the wider workforce to deliver quality Healthcare.
This can use sources from outside but must be based on UK and NHS care.
I would like the gap analysis to be based on the management of the population of Barking, Havering, & Dagenham East London; specifically focusing on the management of Non-Alcoholic Fatty liver disease and the NICE Quality Standards that should be met for this group;
It needs to include a literature review and must have a gap – analysis comparing our current performance with where we should be.
It must reflect on current recommendations Currently we have 160 patients with Non-Alcoholic Fatty liver disease.
And none of these have recorded in their records an annual fatty liver review.
As per the NICE recommendations above; I would like it to be focusing on how we can ensure we have documented in patients records on an annual basis;
LFT Blood test, weight, BP, alcohol use, and either Fib4 score or NAFLD score.
We are not meeting this need standard currently.
Would need to discuss barriers for getting this done; patient engagement, recall process, need for a blood test specifically for this purpose as blood value for fibrosis score required (AST) not on routine bloods
Want to also discuss health promotion in terms of lifestyle, diet, exercises, weight loss, alcohol intake as relevant to NAFLD
Would like to discuss team involved in health promotion; pharmacists, practice nurses, online resources, hospital consultant if referred because high risk fibrosis
Services available; dieticians, exercise programs. Hospital consultant referral.
And must have a section about my own professional development understanding how to be an effective leader and member of a team in identifying a weak area, and motivating team members to introduce and maintain change in current practice. Some ideas of how to improve in the future and how to perhaps improve other areas of care also. and The personal qualities required to do this and challenges
The paper must demonstrate ;
1. Critically appraise their role in Patient advocacy: demonstrating the ability to promote entitlement and to empower service-users to become informed and active participants in their own care.
2. Evaluate and identify through critical gap analysis why co -creation with service users and stakeholders is crucial and how it should be enhanced to provide comprehensive care across pathways.
3. Analyse available evidence to synthesize ideas or information in innovative ways, and generate transformative solutions seeking to continually improve care and support delivered as an individual provider or as a member of an inter-professional team.
