Create a short interview of you talking to your assigned patient or their caregiver. Ask them five questions to get additional information to treat your patient.
Include a rationale for why you want to know this information. Explain your rationale as though the patient asks why you asked them that question. For example, you might say, “Have you fallen? I am asking because a fall may indicate a recent stroke.”
