The following Essay plan could be used:
• Introduction
• Following introduction to the assignment, a brief scenario of the patient with long term condition from recent clinical placement experience should be highlighted.
• A brief discussion of the pathophysiology of the long-term condition should follow.
• (Why did you choose this patient?) For example, diabetes is increasing, justify the selection with reference to epidemiological data. Justification could also be due to personal and professional interests. Significance of the epidemiological data should be examined in sufficient depth. The cost to the NHS should be outlined.
• Identify and explain the nursing assessment framework / model used to assess the patient’s holistic needs, Roper et al (2008). What does Roper et al 2008 mean?
